Important Information to know about using Insurance:

As each insurance plan may vary, we encourage clients wishing to use their insurance to contact their insurance provider directly for an explanation of benefits.

Insurance companies may impose restrictions on the length and type of therapy, so it's essential to verify your mental health coverage with your provider.

Please note that a diagnosis is required to utilized insurance and that insurance companies can ask clinicians for details regarding the clients’ diagnoses, symptoms, behaviors, and treatment plans. I will always notify you if your insurance company requests any treatment documentation.

Additional Payment Information:

If your insurance is with a provider that we are not in-network with, a superbill (a specialized, line item invoice) can be provided to consumers, for submission to their respective insurance companies for possible reimbursement of services. Please note: it is the consumer’s responsibility to follow up with their insurance providers regarding reimbursement of services, out of network policies, and coverage. Care and Counseling Services, LLC is not responsible for insurance reimbursement fees when superbills have been submitted, or for fees associated with providers not accepted by Care and Counseling Services, LLC.

Insurance and Payment Policies

No Surprises Act

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059.
